You’re right, she’s in a mental hospital. An Todoroki explained in episode 6 of that Season that she burned him by pouring boiling water on his face.

The thing is, todoroki’s mom did not want to marry endeavor, not worth mentioning havin sex and many kids from him until she had todoroki who was endeavor’s "masterpiece". After years of abuse she was scared to death, and started to go nuts. She did not burn him out of hatred. IIn the manga is shown better, she only sees his fire side in that moment of extreme terror and panic, that’s why she did that. For a moment she thought it was endeavor again comming to torment her

Todoroki’s mom poured boiling water over him. Also it makes sense for todoroki to be unsure of himself after the midoriya fight. Deku shook his foundations.

Basically she is in a mental Hospital for what she did to him, at first when Endeavor said he put her in a hospital everyone thought that he meant he fucked her up, but turns out he put her in a mental hospital but no…she is not crazy my guys, keep up with the awesome reactions bros

Endeavor pretty much traumatized her with how he treated her. He bought off his mom’s family so he could forcibly marry her and create the perfect fire/ice quirk (Todoroki). Thats why Todoroki wont use his fire because of how bad endeavor was to his mom and him.

I don’t know why they had to change the wording for Todoroki’s goal in the dub. In sub and the manga Todoroki calls saving his mother what will be his STARTING LINE. This connects him to his other two rivals Midoriya and Bakugou in that they already had their STARTING LINES in season 1.

They did tell how Todoroki’s burn happened. Todoroki told Deku in S2E6 that his mom threw boiling water on his face. And they showed the scene in the flashback in S2E10, just not the act itself. It’s reasonable to leave those horrible acts vague. It’s not an adult show after all.
